Telehealth informed consent for care delivered by Restorify Health.

What Telehealth Is
Telehealth means receiving health care from a licensed provider using secure electronic tools — online forms, messaging, phone or video — instead of an in-person visit. Restorify Health is a telehealth platform operated by Ream Franchise Group LLC. Clinical care is provided by licensed providers of LocumTele, an independent medical practice.
Scope of Services
Through this site you can request a provider evaluation for medical weight management and menopause and hormone care. A licensed provider decides whether any treatment is appropriate. Some services are not available in every state; see State Availability.
Nature of the Provider Relationship
Completing an intake does not by itself create a provider-patient relationship. A relationship begins only when a licensed provider reviews your information and accepts you for care. Your provider is a nurse practitioner or physician assistant licensed in your state and working under the clinical oversight required by that state.
Potential Benefits
Telehealth can make care easier to access, reduce travel and waiting time, and allow ongoing follow-up with your provider from home.
Potential Risks and Limitations
Your provider makes decisions without a physical examination, which may mean a condition is missed. Incomplete or inaccurate answers may lead to an inappropriate recommendation. Technology failures can delay care. Despite security safeguards, electronic information could be accessed by an unauthorized person. Any prescribed treatment can cause side effects; these are handled through follow-up rather than in person. Your provider may decide telehealth is not right for you and refer you to in-person care, testing or another clinician.
Alternatives to Telehealth
You may choose an in-person visit with a local clinician, urgent care, or not to seek treatment.
Your Responsibilities
Give complete and truthful information, including all medications, supplements, conditions, pregnancy and breastfeeding. Tell your provider about any change in your health or side effects. Be physically located in a state where your provider is licensed at the time of each interaction. Follow your provider’s instructions and the prescription label.
Privacy and Security of Telehealth Communications
Your information is transmitted and stored using encrypted, access-controlled systems and handled as described in our Notice of Privacy Practices.
Recording and Documentation
Your provider documents your care in a secure medical record. Consultations are not recorded unless you are told in advance and give separate consent.
Prescribing and Pharmacy
If your provider prescribes treatment, the prescription is sent to a state-licensed pharmacy that dispenses and ships it to you. Some states require a live audio or video visit or an additional consent before a prescription; your provider will follow your state’s rules, which may add a step or mean care cannot be provided in your state. Prescribing is never guaranteed.
Emergencies
Telehealth is not for emergencies. Call 911 or go to the nearest emergency department for chest pain, trouble breathing, a severe allergic reaction, thoughts of self-harm, or any severe or rapidly worsening symptom.
Right to Withdraw Consent
You may withdraw this consent at any time by writing to support@restorifyhealth.com. Withdrawing does not affect care already provided, but care may not be able to continue through this platform.
Consent Acknowledgement
By checking the consent box, you confirm you have read and understood this consent, had the chance to ask questions, and agree to evaluation, treatment and follow-up by telehealth, including review of your written intake.
